USP 795 COMPETENCY ASSESSMENT HAND HYGIENE

Review USP 795 guidelines for compounding

Wash hands with soap and water for at least 20 seconds

Use hand sanitizer with at least 60% alcohol if soap and water are not available

Ensure nails are clean and trimmed

Remove jewelry before compounding

Use nail brush to clean under nails

Apply lotion to hands to prevent dryness

Use paper towels to dry hands thoroughly

Avoid touching face or other surfaces after washing hands

Follow proper hand hygiene techniques as outlined in training

Wear gloves when handling chemicals or medications

Wear a mask and eye protection when compounding hazardous drugs

Change gloves between each step of compounding

Wear a gown to protect clothing from contamination

Use a face shield when handling hazardous drugs that may splash

Ensure all PPE is properly fitted and in good condition before beginning compounding

Remove and properly dispose of all PPE before leaving the compounding area

Wash hands thoroughly after removing PPE

Label all containers with appropriate information

Use aseptic technique when compounding sterile medications

Store compounded medications in appropriate containers

Clean and disinfect compounding area regularly

Verify accuracy of ingredients before compounding

Follow proper calculations and measurements when compounding medications

Dispose of any expired or unused medications properly

Check for any signs of contamination during compounding process

Follow proper hand hygiene before, during, and after compounding medications

Document all compounding activities

Record any deviations from compounding procedures

Keep records of all medications compounded

Ensure all staff members are trained on proper hand hygiene and compounding procedures
