Record any observations or incidents in client's chart

Ensure all documentation is accurate and up-to-date

Assist client with any final tasks before bed

Ensure client is comfortable and settled for the night

Communicate any important information to next shift or supervisor before leaving

Any additional tasks requested by client or supervisor

Any additional support needed for client's comfort and well-being

Any other duties as assigned by supervisor

Follow up with supervisor or client's healthcare team as needed

Provide feedback on evening routine and any suggestions for improvement

Ensure client's needs are met and any concerns are addressed before end of shift.
