Confirm patient identification

Verify the correct procedure and site with the patient and healthcare team

Review the patient's medical history, allergies, and any relevant lab results

Assess the patient's coagulation status and document it

Ensure that necessary supplies and equipment are available and in working order

Confirm that the patient has provided informed consent for the procedure

Discuss the benefits, risks, and alternatives of the procedure with the patient

Address any questions or concerns the patient may have

Perform a thorough physical assessment of the patient

Evaluate the patient's vital signs and document them

Assess the patient's level of discomfort or pain

Evaluate the patient's mental status and ability to cooperate

Assess the patient's veins for suitability for central venous catheter or PICC line insertion

Determine the appropriate site based on patient's condition and treatment plan

Consider any contraindications or potential complications associated with specific sites

Cleanse the insertion site with an appropriate antiseptic solution

Allow the antiseptic to dry completely before proceeding

Apply sterile drapes to create a sterile field around the insertion site

Cover the patient with a sterile drape to maintain aseptic technique

Gather the entire healthcare team involved in the procedure

Verify patient identification, procedure, and site once again

Confirm the patient's understanding and consent

Address any concerns or questions raised by the healthcare team

Ensure that everyone is prepared to proceed with the procedure

Perform hand hygiene and don sterile gloves

Use appropriate sterile technique during catheter insertion

Monitor the patient's vital signs and response to the procedure

Administer local anesthesia as necessary and document it

Follow proper catheter insertion guidelines and secure the line appropriately

Document the details of the procedure, including the catheter size, insertion site, and any complications

Observe the patient for any signs of infection or complications

Educate the patient on proper care and maintenance of the catheter or PICC line

Provide the patient with written instructions and contact information for any concerns or questions

Schedule necessary follow-up appointments or assessments

Arrange for appropriate dressing changes and maintenance of the line

Communicate with the patient's healthcare team about the procedure and any related findings

Document the patient's response to the procedure and any further interventions required
