Introduction
Gather a team of individuals with expertise in various aspects of the process
Define the scope and boundaries of the analysis
Preparation
Compile all relevant documentation such as process flow diagrams, P&IDs, and operating procedures
Review previous incident reports and near misses
Identify potential hazards based on the process design and operating conditions
Hazard Identification
List all possible hazards associated with the process
Consider external factors that could impact the process (e.g. weather, neighboring facilities)
Use techniques such as What-If analysis, Hazard and Operability Study (HAZOP), and Fault Tree Analysis to identify potential hazards
Risk Assessment
Evaluate the likelihood and consequences of each identified hazard
Prioritize hazards based on the level of risk they pose to personnel, the environment, and the facility
Determine if existing safeguards are sufficient to mitigate the identified risks
Recommendations
Develop recommendations to reduce or eliminate the identified hazards
Consider engineering controls, administrative controls, and personal protective equipment as potential solutions
Document all recommendations and assign responsibility for implementation
Follow-Up
Monitor the implementation of recommendations
Conduct regular reviews of the process to ensure that new hazards are identified and addressed
Update the Process Hazard Analysis as needed to reflect changes in the process or operating conditions.
