Lockout tagout (loto) checklist

Preparation

  • Locate the specific machine or equipment requiring maintenance.
  • Ensure the correct identification of the equipment model and serial number.
  • Verify that the equipment is clearly labeled and documented.
  • Cross-check with maintenance logs for accuracy.
  • Obtain the energy control procedure documentation.
  • Understand the specific types of energy sources involved.
  • Identify the isolation points for each energy source.
  • Review any specific steps or precautions mentioned in the procedure.
  • Gather all affected employees before starting the LOTO process.
  • Explain the purpose and importance of LOTO to employees.
  • Inform them about the duration and expected impact of the LOTO.
  • Ensure employees have an opportunity to ask questions.

Energy Isolation

  • Review equipment manuals and diagrams.
  • Identify all potential energy sources (electrical, hydraulic, pneumatic).
  • Check for stored energy in springs, capacitors, etc.
  • Label each energy source for clear identification.
  • Follow the manufacturer's shutdown instructions.
  • Use control panels or switches to power down.
  • Ensure all operators are aware of the shutdown process.
  • Allow the equipment to come to a complete stop.
  • Use lockout devices on energy isolating devices.
  • Apply tags that indicate maintenance work ongoing.
  • Verify isolation by attempting to operate controls.
  • Ensure all sources are de-energized before proceeding.

Lockout/Tagout Implementation

  • Identify all energy isolating devices associated with the equipment.
  • Select appropriate lockout devices for each energy source.
  • Physically apply each lockout device to prevent operation.
  • Confirm that all devices are properly engaged and functional.
  • Prepare tags with clear, legible information.
  • Include the reason for lockout and the name of the person locking out the device.
  • Securely attach each tag to its corresponding lock.
  • Ensure tags are visible and easily readable.
  • Verify that all locks are properly engaged and cannot be removed.
  • Conduct a visual inspection to confirm no unauthorized access.
  • Communicate lockout procedures to all personnel in the area.
  • Maintain a log of all personnel involved in the lockout process.
